4D-Flow MRI Characterization of Pulmonary Flow in Repaired Tetralogy of Fallot

چکیده

Patients with Tetralogy of Fallot (TOF) have multiple surgical sequelae altering the pulmonary flow hemodynamics. Repaired TOF (rTOF) adults frequently develop regurgitation impacting blood pressure, right ventricle load, and We aimed to evaluate hemodynamics using 4D-flow magnetic resonance imaging (MRI) for characterizing altered flow, viscous energy loss (EL), wall shear stress (WSS), pressure drop (PD), ventricular analysis (VFA) in rTOF patients. hypothesized that based parameters can identify alterations. A total 17 patients (age: 29 ± 10 years, 35% women) 20 controls 36 12 25% were scanned a dedicated cardiac MRI protocol. Peak velocity regurgitant fraction significantly higher (p < 0.001). WSS was consistently elevated along PA ≤ 0.05). The average circumferential than axial at main artery PD EL as compared For VFA, delayed ejection increased retained inflow decreased To conclude, this study demonstrated exhibit peak velocity, valvular regurgitation, WSS, EL, PD, VFA.

Analysis of right heart flow patterns in repaired Tetralogy of Fallot with 4D flow-sensitive MRI

Introduction Cardiac MRI (CMR) is used to follow patients after TOF repair to assess pulmonary regurgitation (PR), pulmonary stenosis (PS) and right ventricular (RV) function. 4D flow-sensitive MRI techniques enable visualization of complex flow patterns [1],[2]. With the ability to simultaneously acquire morphology and hemodynamics for visualization and quantification, they may improve evaluat...

Right ventricular kinetic energy: 4D flow MRI analysis of healthy volunteers and repaired Tetralogy of Fallot

Background Repaired Tetralogy of Fallot (rTOF) is currently monitored with Cardiovascular MR (CMR) to measure right ventricle (RV) volumes and function. However, these indices do not always reflect performance status or predict success following repair of pulmonary insufficiency. RV kinetic energy (KERV) may provide a more robust measure of RV efficiency than RV end diastolic volume (EDV) index...

Enhanced segmentation improves 4D blood flow quantification in patients with tetralogy of Fallot and pulmonary regurgitation

Background 2D phase contrast (PC) MRI provides reliable quantification of blood flow in pts with tetralogy of Fallot (TOF). While 2D PC is the standard for evaluating pulmonary regurgitant fraction, it is limited by single direction velocity measurement and may be inadequate to characterize 3D hemodynamics. Multiple planar acquisitions are often required. While 4D flow MRI provides simultaneous...

Residual problems with repaired tetralogy of fallot.

Operable Problems Pulmonary regurgitation and RVOT stenosis are anatomical complications that can be relieved only by anatomical repair including surgery and percutaneous catheter treatment.4,5 Therefore, appropriate timing of such intervention is important. However, definite criteria for reoperation are not yet determined. There were several advocated criteria for re-intervention using magneti...

Uteroplacental Doppler flow and pregnancy outcome in women with Tetralogy of Fallot: Uteroplacental flow in women with Tetralogy of Fallot

Objective Pregnancy in women with surgically corrected tetralogy of Fallot (ToF) is associated with cardiac, obstetric and neonatal complications. We compared uteroplacental Doppler flow (UDF) measurements and pregnancy outcome in women with ToF and in healthy women and aimed to assess whether a relationship exists between cardiac function and UDF in women with ToF. Methods We evaluated prospec...
